By accepting below, I allow Westlake Bulldogs Youth Organization (otherwise referred to as "Westlake Bulldogs") and its affiliates, up to and including Board Members, to perform criminal history research on my person.

I acknowledge and understand that this may include but is not limited to, formal criminal history check performed by the Maryland State Police, research on Maryland State Police National Registry for Sex Offenders, and further research conducted by a Westlake Bulldogs designated Board Member or Research Committee.

I acknowledge the information obtained is used for and in conjunction with my submitted application for member to the said, Westlake Bulldogs Youth Organization.
I acknowledge, upon obtaining any information stating conviction of any of the following, Westlake Bulldogs will deny/ decline my application for membership without any further due process.

1. Violent Crime Against Minor(s),
2. Sex Offense against a minor(s),
3. Violent Sex Crime

I acknowledge information obtained stating conviction of felonious acts other than stated above, may require further review and interview of said, Westlake Bulldogs Board of Directors.

By signing said agreement, I pledge to inform a member of the Westlake Bulldogs Board of Directors within 24 hours of any said changes in criminal status. This is to include but not limited to Maryland State Law conviction of misdemeanor, felony and/or sex offense charges.

Information obtained will be kept confidential and will not be released to any other parties, unless strictly in compliance with applicable laws.

Coaches\Members Agreement:


The following conditions are MANDATORY for all coaches\members:

All coaches involved in Westlake Bulldogs are required to attend a mandatory preseason coaches meeting after approval by Westlake Bulldogs Board of Directors.

All coaches must turn in an updated application to the Board of Directors every year.

All funds collected in Westlake Bulldogs name will be turned into the Westlake Bulldogs Treasurer.

I understand that there may be a mandatory officials meeting for which I or one of my staff must attend.

I understand that I must have a representative at all monthly meetings called by the Board of Directors. Representatives can be Head or Assistant Coaches, or a person designated by you.

I understand that as the Leagues representative, in direct association with the players and cheerleaders, I am responsible for the maintenance, care, and turn in of all equipment. In those cases where equipment is not collectible, it will be Westlake Bulldogs responsibility to collect financial compensation from the family of the player(s) involved.

All coaches will be available for work in the equipment room, if required, before and during the equipment issue as per Equipment Coordinator procedure/schedule (changes can be made via Equipment Coordinator).

All coaches will help to maintain order on and around the Westlake Bulldogs areas and while coaching maintain player/spectator 15-yard boundary.

All coaches will insure field and are clean up after each game (entire spectator side past end zones and behind stands). This is required to insure field availability for future years.

Each team is required to turn in the results after each game win or loss, either written or verbal to the Cheer Director within 24 hours after the game. This is required as part of our obligation to our sponsors and players.

All coaches will encourage good sportsmanship and conduct themselves in only the best exemplary manner both on and off the field in regards to Westlake Bulldogs and its participants.

All coaches will read and accept the COACHES CODE OF ETHICS.

COACHES\Members CODE OF ETHICS
SPORTSMANSHIP. All members of Westlake Bulldogs, coaches, officials, parents, players, cheerleaders and spectators are required to adhere to the principles of good sportsmanship at all times. Coaches are responsible for the conduct of their players, cheerleaders, assistant coaches and sidelines. Game officials are responsible for enforcing sportsmanship rules for game participants. Persons in violation of this rule may be removed from the area and referred to the Board of Directors for additional actions as deemed necessary. Any person violating any of the rules contained herein may, in addition to penalties enforced by game officials, receive sanctions or actions as determined by the Board of Directors.

I will never verbally abuse any player; doing so will result in immediate suspension.

I will never physically abuse any player; doing so will result in permanent suspension.

I will place the emotional and physical well being of my players ahead of any personal desire to win.

I will remember to treat each player as an individual, remembering the large spread of emotional and physical development for the same age group.

I will do my very best to provide a safe play situation for my players.

I will not recruit out of the clubs designated boundaries.

I will do my best to organize practices that are fun and challenging. I will ensure all players are active participants in all practices.

I will insure that I am knowledgeable in the rules of the sport that I coach, and that I will teach these rules to my players. I will not teach or instruct players tactics or actions which are not within the rules.

I will use those coaching techniques appropriate for each of the skills that I teach.

I will not discredit my team or myself by running up the score on an opposing team.

I will remember that I am a youth coach, and that the game is for children and not adults.

I understand my failure to abide by the Westlake Bulldogs Coaches Code of Ethics can and will result in disciplinary actions by the Westlake Bulldogs Board of Directors up to and including disbarment from all Westlake Bulldogs activities.

The Head Coach shall be responsible for the safety and conduct of their players or cheerleaders and are responsible for the conduct of their assistant coaches during Westlake Bulldogs activities. The Head Coach will assist the game officials in keeping order when requested to do so.

Only head coaches may seek clarification on a call or rule.

You also confirm that:

1. You have read, understood and signed voluntarily the Westlake Bulldogs Coaches Release Form regarding Sex Offenses and further understand any such felonious acts committed during your coaching tenure will immediately negate your coaching contract with Westlake Bulldogs.
2. To abide by the rules and regulations of Westlake Bulldogs Youth Organization, Inc (Westlake Bulldogs).
3. That the game of Cheer, full contact, is based on strenuous physical activity and competition.
4. That you assume the risk of injury and will not hold liable Westlake Bulldogs for any resulting injury before, during or after the game as caused by you or another participant.
5. That to the extent possible you will exhibit common sense and will not purposely seek injury on or off the field.

Westlake Bulldogs will review your application as appropriate and require you to physically sign a printed copy of all agreements previously read. Hard copies will be available and must be signed at the next subsequent Board of Directors meeting or at another time, as agreed to by you and the Westlake Bulldogs Cheer Coordinator.

At that time you will be asked to provide a Government Issued, photograph ID (Drivers License, military or passport) to verify identity.
